Sefton Transition Policy 

In Sefton we understand that transition is a process that requires time and preperation. We know that a successful transition for a child, either when starting nursery or school, sets them off to the very best start. 

It is vital that all professionals working with children in the Early Years Foundation Stage have a clear understanding of their responsibilities in relation to transition. 

The following policy has been developed in collaboration with a number of professionals and parents to support consistent and effective transition planning for all children.
